Peninsula Community Health Services are partnering with us to provide well child & sports physicals on Friday, August 15th from 9:00AM-3:00PM. There is a QR Code on the attached document that you can scan to schedule an appointment. Please note that appointment times are approximate. Scheduled appointments are highly encouraged due to walk-in availability being limited.
