For any student who would like to participate in a sport, please make sure you have a sports physical uploaded into your FinalForms account before the start of practice. If we do not have an acceptable physical, you will not be able to participate.
New to CKMS and need to create a FinalForms account for you and your student? Start an online profile on
FinalForms. Once you are logged into FinalForms, you (and your student) can sign off on the important documents, upload physicals and doctor's notes, and sign up for any sport or club we have to offer.
If you have any problems uploading documents, please bring them to the ASB office any time after we open back up on August 17th.
Anyone wishing to participate in a sport or club for the 26-27 school year will need to purchase ASB privileges. It is a one-time payment of $15 during the school year that will cover the participation fee for any sports/clubs your student wishes to do.
